Incomplete Grade Request

If faced with an emergency such as a severe illness that prevents the completion of a course within the semester, students may request an Incomplete using the Incomplete Request Form

TO BE ELIGIBLE FOR AN INCOMPLETE

  • Must have successfully completed at least 70 percent of the coursework of the affected course
  • Instructor will accept or deny the request and may request documentation

TO APPLY FOR AN INCOMPLETE:

  • Fill out the Incomplete Request Form and send it to your instructor
  • Typed signatures will be accepted if sent from a DU email address

IF APPROVED:

  • The instructor will indicate on the form what work needs to be submitted and the due date
  • Form will be sent to the Associate Department Chair and the student
  • The Associate Department Chair will send the form to academicoperations@davenport.edu
  • A grade of (I)ncomplete will be recorded on the grade report
  • A copy of the form will be added to the students official academic record

IF DENIED

  • The instructor will indicate on the form the rationale for denying the request
  • Form will be sent to the Associate Department Chair and the student
  • The Associate Department Chair will send the form to academicoperations@davenport.edu
  • Form will be added to the students official academic record
