| Certificate of No-Fault Insurance | All vehicles must have a State of Michigan Certificate of No-Fault Insurance inside the vehicles at all times. Please downloaded and print the attached certificate before your trip. Failure to have this certificate in the vehicle may jeopardize your trip. | Download |
| Vehicle Inspection Form | Before you charter or rent a vehicle from DU, you must fill out an provide the Vehicle Inspection Form. Failure to fill out this form may jeopardize your trip. This form must be filled out prior to ANY DU rental regardless if you have rented from us before. | Download |
| M.V.R. Report | This authorizes DU to to look at your driving record. This must be filled out and turned into the Director of Transportation before driving any DU vehicle. | Download |
| Release and Waiver of Liability | The Release and Waiver of Liability form releases DU of any legal repercussions should an injury occur or property be damaged. This must be filled out and turned into the Director of Transportation before driving any DU vehicle. | Download |
